Prospective Parents - Forms
If you are interested in joining the Alliance Academy for the 2020-21 school year please take a look at the slide show (click on Alliance Academy above to go to the main Alliance page) for an overview of how our program works. If you decide to enroll and currently attend a YC school we just need the enrollment form and the curriculum selections forms. If you live outside the district we will need all the forms below.

If you live outside the Yamhill Carlton School District your resident district must approve your students' transfer to our district for enrollment. Please contact your district to obtain the transfer forms and authorization.
